Medical Social Worker (LMSW/LCSW)

We are seeking a compassionate and motivated Medical Social Worker to join a fast-paced hospital setting supporting patient care and discharge planning. This role is ideal for an LMSW or LCSW who is comfortable working directly on a hospital unit alongside a multidisciplinary team. New LMSWs are encouraged to apply if they are confident working in an acute care environment rather than a traditional office setting.

Responsibilities

• Provide social work support to patients within a hospital/unit setting
• Manage discharge planning for approximately 25 patients per day
• Collaborate closely with Nurse Care Managers (NCMs), physicians, and interdisciplinary teams to coordinate safe transitions of care
• Assess patient psychosocial needs and identify appropriate community resources and support services
• Document interventions and maintain accurate patient records
• Participate in multidisciplinary treatment planning and care coordination
• Opportunity to work within pediatric populations if interested

Requirements

• Active LMSW required (LCSW strongly preferred)
• Comfortable working in a hospital or inpatient setting
• Strong communication, organizational, and care coordination skills
• Ability to manage a high patient volume in a fast-paced environment

Preferred Qualifications

• Minimum of 2 years of experience in medical social work, mental health, or human services as part of a multidisciplinary treatment team
• Prior hospital or inpatient experience strongly preferred
• Candidates with outpatient or nonprofit-only backgrounds may not be the best fit for this role

Schedule

• Monday - Friday
• 8:30 AM - 4:30 PM or 9:00 AM - 5:00 PM
